Veterinarian Information
At the time of purchase you will receive a form similar to this, but specific to your puppy. Please provide your chosen veterinarian with this form to help them provide your puppy with the best care from the very beginning.
Puppies & Parasites
This form provides you with general information. The buyer will sign this form at the time of purchase acknowledging their understanding of the information therein.
*If you are unable to attend our companion connection day then we will set up a zoom meeting for you to select your puppy so that you do not lose your decision spot.
**We offer full or partial delivery in NY and neighboring states more an additional fee.
